Additional Information:

  1. This order form will be used to quote the project as well as to be used as a contractual agreement. This document will not be legally binding and considered a contract until both parties sign. “Both parties” is defined as a representative of the couple and an agent of WeddingMusicWorks.
  2. 50% down payment at time of signed contract is required.
  3. Balance is due prior to shipment.
  4. Lead-time will range from 4 to 6 weeks for Proposals 1 & 2.
  5. Lead-time will range from 6 to 8 weeks for Proposals 3 & 4.
  6. Lead-time for Proposal # 5 TBD.
  7. Quote will be presented following completion of the order form.
  8. Project will not start (Graphic artwork, CD Duplication, etc.) until both parties sign off that there is an agreement of what is to be expected.
  9. Shipment will be by US Mail unless otherwise specified.
  10. Shipping costs are extra.
